Founded more than twenty years ago, our facility has an outstanding reputation and is located in a quiet residential area in the Evergreen neighborhood of San Jose, California. GRACE GARDEN is LICENSED to provide care for 6 elderly. We pride ourselves in providing more personalized care and have the highest caregiver to resident ratio to care for your loved ones.
Caring and Supportive
We understand that not one care plan fits all. Daily care for your loved one can include anything from meal preparation, hygiene, cleaning, medication administration, activities and supervision. We will take the time to get to know the residents need and develop an individualized care plan that fits their specific needs.

We provide one-on-one attention and care that cannot compare in other settings. We have a combined 70 years experiences providing care for our families and senior residents.

- Coordination of care with medical professional and other service providers (doctors, nurses, hair salons , toenails, etc.)
